ADOPTION APPLICATION

 

1. Do you have a securely fenced yard for the pup?

 

2. How often would the pup be left alone and for how long?

 

3. Do you have dogs of other breeds? If so what breeds?

 

4. Have you ever raised a puppy? If not, this is not a deal breaker.  I would just like you to know that I can help you with the process.
